Case Manager - Rapid Rehousing
Purpose
The purpose of this position is to provide direct advocacy and case management services to eligible clients for Rapid Rehousing. This position has responsibility related to assessments, intakes, housing search and coordination of community resources and referrals for clients.
Job Responsibilities
*Assesses needs (met and unmet), abilities, strengths, barriers and preferences of individuals seeking services and support through in-take assessment interview
*Client development of individualized service plans
*Utilize the case management model which is strength based with a housing stability focus through progressive engagement
*Familiarity with the Housing First Philosophy, harm reduction
*Secure, maintain and conduct regular reviews of program and statistical records through agency and funder databases
*Provide planning, implementation and collaborative internal and external supportive services assistance in the following areas: financial education, permanent housing stability, health and wellness, and other areas as appropriate
*Participate in all required organizational, program specific and position related professional development
*Meet with external collaborators and members of community coalitions and Continuum of Cares
Job Requirements
*Must be sensitive and responsive to the cultural differences in the organization's service populations
*Proficiency in Microsoft programs (2013) and the following databases; Homeless Management Information Systems (HMIS) and Efforts to Outcomes (ETO)
*Database experience; strongly preferred
*Strong verbal and written communication skills
*Ability to remain calm and responsive while de-escalating crisis situations; critical thinking, analytical, problem solving and negotiating skills
Education/Experience Requirements
Graduate degree in Social Work required
Previous experience working in a nonprofit organization with direct client services experience for high risk populations experiencing homelessness or at-risk of homelessness, substance use, mental health and chronic health
Experience working with survivors of domestic violence and/or sexual assault including dating violence, stalking, human trafficking, or other dangerous or life-threatening conditions that relate to violence against the individual or a family member, including a child
Experience in community networking, case coordination for multi-disciplinary teams
Experience in implementing the Vulnerability Index Services Prioritization and Decision Assistance Tool (VI-SPDAT and full service Prioritization Decision Assistance Tool (SPDAT); preferred
